It is required that applicants are currently in or have completed an accredited fellowship training program in Interventional Cardiology, Vascular Surgery, Interventional Radiology, or Cardiothoracic Surgery. Malpractice Insurance Coverage:
All fellows are required to possess malpractice insurance coverage in the amount of $1,000,000/$3,000,000 professional liability. You will need to provide us with a certificate of insurance verifying coverage during your training. In the event that your current provider will not extend coverage during your fellowship we can help you obtain coverage during your fellowship. Please click here to download an application. Illinois Physician License:
All fellows must obtain an Illinois Physician license prior to start date of their fellowship. Applications for temporary or permanent Illinois Physician licenses can be downloaded from the Illinois Department of Financial and Professional Regulation website (www.idfpr.com/Renewals/apply/Physician.asp). Saint John’s Hospital Credentials:
All fellows must become credentialed to practice at Saint John’s Hospital in Springfield Illinois; and a formal letter of request must be submitted in order to receive the St. John’s Hospital pre-application for appointment. Sadie Samson will facilitate the submission of this letter to the hospital on your behalf. Please contact her at email@example.com to start the process, or if you have any additional questions. * Please note: There is a $250.00 fee associated with your application for appointment, which will be addressed once the application process has begun.
Prairie Advanced Heart & Vascular Fellowship Tuition
The tuition is $10,000 each month. A non-refundable deposit of $10,000 is required at the time of acceptance to secure your position. The remainder is due upon matriculation.
